What is Bridges Home Living?
Bridges Home Living helps people with disability to explore and make decisions about:
- Where they want to live
- What type of home they would like to live in
- Who they would like to live with
- How they would like to get involved with their local community
- What types of support they need and would like to use
- Costs for providing support
A person with disability, their family and important people in their lives are central to exploring and establishing a new home and the right supports. This service is offered as a partnership between Anglicare WA, Foundation Housing and Silver Chain who have combined their expertise to offer an individualised service for each person.
Through Bridges Home Living, people can be assisted to find a home, manage housing tenancies or home ownership, coordinate daily support within their home and local community and access additional specialised health and home support when needed.
Bridges Home Living staff also work with other organisations and Anglicare WA services to offer ensure recreational, training, work and volunteer activities are available. This includes the Bridges Program.
Who is Bridges Home Living for?
Bridges Home Living is for people with disability who have individual funding available and who wish to establish supported living arrangements based on their needs, wishes and interests. In 2014, Bridges Home Living was prioritised for people who wish to move from a Disability Services Commission group home or hostel to individualised living arrangements.
Where is Bridges Homes Living located?
Bridges Home Living operates in the Perth metropolitan area including in and around Mandurah, the south west and great southern regions of WA.
How can Bridges Home Living be accessed?
People with disability and their families can choose to develop a Bridges Home Living option using individual funding provided by the Disability Services Commission. To find out more information contact:

Bridges Home Living is overseen by a reference group including people with disability who have broad experience of individualised disability options.
People with disability who access Bridges Home Living receive support funding from the Disability Services Commission.
Anglicare WA is a member of WA Individualised Services (WAIS) who foster and support the development of person-centred and self-directed services for people with disability.
